DIY paperwork

The Readiness Packet you fill in yourself.

This is the paperwork. It is not a Sentry visit, not medical advice, and not a completed packet.

You print it, fill it, post the snapshot where Fire and EMS look first, and give the family copy to at least two trusted people. If you want us to do that with you in the home, schedule a visit instead.

What you get

  • Emergency Medical Snapshot
  • Family Continuity form
  • Home Safety Checklist
  • Notice & Disclaimers
  • Print-at-home first-responder sticker
  • Texas OOH-DNR information sheet (the DNR form itself is not included)

What you must do

  • Complete the forms with current facts
  • Post the snapshot where crews look first
  • Give Family Continuity to two trusted people — print it; do not email the completed form
  • Walk the checklist and fix what you can
  • Print and place the sticker

What this is not

  • Not a Sentry home visit
  • Not a diagnosis, treatment, or medical record from Sentry
  • Not a promise Fire or EMS will find or use it
  • Not a will, power of attorney, or DNR
  • Not legal advice
